These commitment forms (both administrator and teacher forms) should be filled out by you and your administrator and faxed back to the project using the available fax cover sheet. Make sure that you submit the forms specifically associated with your participation strand (MS - Master's Strand; LS - Leadership Strand; TPD - Teacher Professional Development Strand).
